Supervision of Communication Health Assistant Agreement Form

TO BE COMPLETED BY REGISTRANTS WHO PREVIOUSLY SUPERVISED OR CURRENTLY SUPERVISE CHAs

All registrants of RCASLPNB who supervise Communication Health Assistants (CHAs) must complete and submit the Communication Health Assistant Agreement when commencing and/or ceasing this supervision role.

Registrants supervising CHAs may be audited to ensure compliance with Rule 15.4.2 (d).

- I supervise a CHA and understand that CHAs working in speech-language pathology or audiology must be supervised by a qualified speech-language pathologist or audiologist in accordance with Bylaw 15 and Rule 15.
